When Do I Need a Corrected Title?
There are three instances when you must apply for a corrected title. If your original title is lost, stolen or mutilated, you must apply for a Duplicate Title.
Note: Corrected title applications involving the removal of a name because of divorce or the death of a co-owner or for changing an owner’s name because of marriage must be accompanied with a $15 title fee. Other changes may be made at the same time providing divorce, death of a co-owner or marriage is the prevailing reason for the change.
- WHEN you are removing an owner's name due to divorce, death, etc.
SUBMIT the Application for Vehicle Transaction(s) (VSD 190) with an affirmation from the individual releasing interest in the vehicle or by having them sign the assignment area, or a copy of relevant pages in the divorce decree awarding the vehicle. Divorce decree pages must include the name of the court, identity of the vehicle and signature of the judge.
In the case of a deceased owner, a certified copy of the death certificate must be submitted. If the estate is being probated, a certified copy of the letters of office, administration, testimony or court order must be submitted. More about transferring the title in case of a deceased owner.
- WHEN you are adding or changing an owner's name due to Marriage, or changing an owner's name due as Affirmation of one and the same, or Court Order,
SUBMIT the Application for Vehicle Transaction(s) (VSD 190) with your $65 title fee and a $3 fee to correct the registration card. If your registration sticker expires soon, you may make the appropriate corrections on the renewal application; however, the title and registration must be corrected at the same time.
- WHEN you are adding a lien,
SUBMIT the Application for Vehicle Transaction(s) (VSD 190). Indicate the full name and address of the lienholder.
When seeking to correct information on your title, and the title has been lost, stolen or mutilated, you must apply for a Duplicate Title. However, you may check the box for corrected title if one of the above listed conditions apply.

Fill Out Application
You must fill out the Application for Vehicle Transaction(s) (VSD 190).
- Check the box for Corrected Title in the top section.
- Fill out the sections that are changed.
- Indicate your reason for requiring a corrected title.
- Write in the current vehicle odometer reading.
- Sign the document.

Liens
Illinois titles are mailed to the lienholder if a lien is applied. Include the complete name and address of the lienholder on the duplicate title application. If there is no lien, the title will be mailed to the principal owner listed first on the application or to the address listed in the "Mail To" area.
To Remove Lienholder:
- Submit lien clearance letter from the lienholder indicating that the lien has been paid in full; or
- Submit lien contract stating that the lien has been paid in full;
- Submit a court order directing our office to issue the duplicate title without the lien or lienholder's name.
- Submit a court order directing the Office of the Secretary of State to issue the duplicate title without the lien or lienholder's name. The Court Order must be signed by the Judge residing over the case and/or reflect the FILED stamp of the Clerk's Office and include the year, make and vehicle identification number of the vehicle in question.

Submit Application
You must submit your application, a surrender title, and the $65 title fee to the Secretary of State's Office. Applications will be returned if the information or fee is insufficient.
